2012 Joint Contract Negotiations Survey

NOTE: In the 2012 Joint Contract Survey printed book, question 54 regarding insurance the response “I am married and cover myself and my spouse” was inadvertently left out. This response is available to chose when participating in the survey online or via the phone.

Dear United Flight Attendants,

Preparations are underway for our Single Contract Negotiations, which are scheduled to begin the last week of August with United Airlines management. We need your input to negotiate an Agreement with improvements for all Flight Attendants at the new United Airlines that reflect the invaluable contributions each of us makes to the success of our airline.

We have prepared this survey based on feedback from your Local Council Presidents and the Continental Micronesia and United Master Executive Council Officers. We have also incorporated questions based upon direct feedback from you through phone calls, emails and face-to-face conversations.

Your participation in this survey is critical to our success in negotiations. We rely on your input to direct the course of our negotiations. This survey provides all Members an equal opportunity to communicate your needs and priorities. Our negotiating priorities are determined by the Membership.

The survey will open on July 14 and close on August 5, 2012. A copy of the survey is enclosed in this mailing. We strongly encourage you to take the time to answer the questions on paper prior to entering your responses through the survey on the web or telephone. Review the language of your current Contract and ask your Leadership questions.

Take part in the survey and encourage your flying partners and crewmembers to do the same. Please try to complete the survey online. You have the ability to stop mid-survey if you need to get questions answered, and using the online method provides an easier ability to resume where you left off and then complete the entire survey. We will hold your responses in strict confidence.

We will use the feedback from this survey to formulate our proposals and guide us through negotiations. Feedback from this survey will help us create our opening proposals, also known as an “Opener.” The Opener may be detailed or general, and serves to put management on notice regarding the changes we collectively seek through our upcoming negotiations. There may be additional surveys as we progress through negotiations.

We have a common goal for these negotiations: a Single Collective Bargaining Agreement that combines the best of all three current Contracts. Working together, we will be successful in achieving a Single Contract for all Flight Attendants at the new United Airlines.

Survey Instructions

Survey Opens: July 14, 2012 1200 Noon Central Time

Survey Closes: August 5, 2012, 1700 Central Time

Accessing the Survey

  1. Take survey now >
  2. Dial 1-800-706-7726 to enter your responses through the phone. Entering your responses on-line is preferred since the length of the survey makes telephonic responses difficult.

Login Information:

VIN: Your six-digit file number (use leading zeroes)
Example: If your file number is 5555, then your VIN is 005555.

PIN: Your eight-digit birthdate (MMDDYYYY)
Example: If your birth date is March 1, 1950, then your PIN is 03011950.

Web Survey Instructions

  1. Go to www.unitedafa.org and click on the “2012 Joint Contract Negotiations Survey” link. Enter your VIN and PIN, then click “Login.”
  2. You will now be required to change your PIN to ensure the privacy of your survey responses. Change your PIN to any 4-8 digit number you choose. You must remember your new PIN, because you will need it if you wish to access the survey system again.
  3. To take the survey you may either click the “Take / Resume Survey Now” button, or choose Surveys from the menu.
  4. Mark your response to each survey question. Submit your finished survey by clicking “Submit Survey” button.
  5. Write down your confirmation number – this is your survey receipt – and then click “Logout.”

Save Partially Completed Survey Responses

If you do not have time to complete the survey in one session, you may save a partially completed survey by clicking the 'Save Partially Completed Survey' button. Once saved, you may return at a later time and complete the survey. To resume a saved survey, simply click the “Take / Resume Survey Now” button, or select Surveys from the menu.

If you select the option to “Save Partially Completed Survey,” you will be advised that your responses will not be submitted as part of the survey tally until you click the button to “Submit Survey.”

Session Time Out

The survey system is configured to allow 4 hours before timing out your session. If you need more than 4 hours to complete your survey, simply save your partially completed survey, and then resume. This will begin a new 4 hour timeout session.

Technical Assistance

After you complete the survey, you may change your choices, through either the Internet or the telephone method. To do so, you must use your new PIN to access the system again. Only the last survey response will be received. If you experience any problems with the survey system, please contact the Survey Help-Line at 1-800-624-3907, Ext 545.

Telephone Survey Instructions

  1. Dial 1-800-706-7726. International based Flight Attendants will have the option of using the toll-free International Dear AFA numbers to connect to the survey system. These calls may be placed by International based Flight Attendants from any of the following cities: Frankfurt, 0-800-1015247; Hong Kong, 800-964-443; London, 0800-917-3126; Narita/Tokyo 00531-121081.
  2. Once you reach the survey phone line, you will be prompted for your VIN and PIN.
  3. The phone system will guide you through the process of changing your PIN, selecting the survey, and choosing your answers. Remember your PIN to access the system again.
  4. Use the touch-tone keypad on your telephone to enter your responses to respond to the spoken questions. Open-ended responses or comments will offer you choice of “No Comment” and “Comment.” If you wish to record a response, select “Comment.”
  5. Once the survey is completed you will receive a confirmation number to be saved as your receipt.
  6. If you hang-up or get disconnected prior to completing your survey, the system will save the survey responses you already answered. You may call back and the system will prompt you whether to continue or restart the survey.

Note: For Internet open ended questions, there is a maximum of 5,000 characters. The phone allows up to 90 seconds. Should you wish to add more comments, please contact your Local Council Office.

Once you have completed the survey, encourage your flying partners to take part in the survey. Remember, the survey closes on Sunday, August 5th at 1700 Central Time.
